Dear Customer
Ability Centre has been advised by the Commonwealth Department of Health that from Monday 22 February 2021, our Supported Independent Living customers will be offered the COVID-19 vaccine.
The COVID-19 vaccine is free and voluntary. The Department of Health will advise Ability Centre of the vaccination schedule for our homes.
Customers will need to have two doses of the vaccine – one initially and then a second at least 21 days after the first dose.
The vaccinations will be administered by Aspen Medical who have been appointed by the government. We are working with Aspen Medical to provide close to home, or in-home locations for the vaccinations. Once we know the details for your vaccination, your Coordinator will call to discuss the arrangements with you and will send you the consent form.
If you would like to receive the vaccination, you need to sign the consent form and return it to your Coordinator prior to your vaccination date. You may wish to discuss any concerns with your GP prior to giving consent.
Your relevant medical information will be shared with Aspen Medical on the vaccination day. Your Coordinator and an Ability Centre Health Lead will be present to support you at the appointment and will have your Profile Book, current medication list and any other relevant information for review.
- Read the following documents:
– Information on the COVID-19 Pfizer (Comirnaty) vaccine
– What to expect on COVID-19 vaccination day - Respond quickly when your Coordinator calls you to let you know your vaccination date and location details. If you do not wish to receive the vaccination, let your Coordinator know at this point.
The Ability Centre team will be working closely with the Department of Health to ensure the vaccination program runs smoothly. Please contact your Coordinator if you have any queries. You may also find it helpful to review the Department of Health dedicated web page for people with disability.
Yours sincerely
